Introduction

The Aesthetic Nurse provides a service to members of the public that require non-surgical treatment.  We operate a three-step process whereby we will require information from you as we are audited to ensure that our ethical throughout. Your information can be collated in person or via online booking/enquiry form. 

The three-step process consists of the following:

Step 1

Complete our consultation documents that includes the following:

Name
Address
Telephone / Mobile
Email address
Date of Birth (we can only allow customers over the age of 18)

Temperature check (to meet the Covid requirement before you can enter the building so we can refuse treatment should you have any signs/symptoms of the virus)

Next of kin details

Medical details 

Your answer to medical questions 

Any other information that you may wish to share whilst in our consultation will be noted on your file 

Step 2

Non-surgical procedure detailed notes from the procedure carried out that will include time and date.

Photographic evidence maybe required for your files.

Step 3

Aftercare consultation following the procedure which a customer will need to adhere to so a signature will be required to confirm that you have understood. All of customers additional questions will be documented to ensure that the customer is happy.

All the information that we have allows us to carry out the procedure under the UK Medical Regulations 

What we do with the information

We store your information on a medical industry server that is highly secure however we do have several paper documents which we store in a lockable cabinet. The property is alarmed so access is restricted as require by Business Insurance purposes.

We would keep your information for as long as the Medical Regulators require us to do so especially if the information is required for legal purposes.

Your data protection rights

Under data protection law, you have rights including:

Your right of access – You have the right to ask us for copies of your personal information. 

Your right to rectification – You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing – You have the right to ask us to restrict the processing of your information in certain circumstances. 

Your right to object to processing – You have the the right to object to the processing of your personal data in certain circumstances.

Your right to data portability – You have the right to ask that we transfer the information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
